Service Policies & Terms of Appointment

By booking an appointment with GeneScreen Mobile Diagnostics, you agree to the following terms and conditions. These policies are in place to ensure efficient scheduling, fairness, and high-quality service for all clients.
- Travel & Service Fees
Multiple State Collections
DNA collections conducted across multiple states will incur a $120 fee per additional state.
This fee also applies to any single collection location that is 61 miles or more from the mobile collection hub, 4319 Covington Hwy. Decatur, Ga 30035.
Multiple Collection Locations
If testing requires multiple collection sites, a Convenience fee of $20 plus a travel fee of $1 per mile for the location that is furthest from the mobile collection Hub, 4319 Covington Hwy. Decatur, Ga 30035, will apply.
Multiple Appointment Times (Same Location)
If multiple parties are scheduled at the same location on the same day but with appointment times more than two (2) hours apart, a $20 Convenience Fee will be charged.
Travel Fee Waiver
Travel fees will be waived only if all of the following conditions are met:
- All parties are tested at the same location
- The location is within 20 miles of the mobile collection hub, 4319 Covington Hwy. Decatur, Ga 30035
- All appointment times are scheduled within two (2) hours of each other
Immigration DNA Cases
All immigration-related DNA collections are exempt from travel fees, regardless of distance.
- Scheduling Requirements
Separate Testing Appointments
To maintain chain-of-custody integrity and ensure compliance standards:
- Parties must not be tested simultaneously
- Appointment times must be scheduled at least 30 minutes apart
Failure to follow this requirement may result in delays or rescheduling.
- Rescheduling Policy
- Clients are permitted one (1) reschedule
- Reschedule requests must be made at least 24 hours prior to the scheduled appointment time
- Requests made less than 24 hours in advance may result in forfeiture of the appointment
- No Refund Policy
All payments are final and non-refundable.
Due to the nature of scheduling, travel commitments, and administrative preparation, no refunds will be issued under any circumstances.
Clients are strongly encouraged to contact us with any questions prior to booking to ensure the correct service is selected.
- Appointment Conduct & Attendance
- Only individuals being tested are permitted at the appointment
- No additional guests, children, or observers are allowed
- All parties must arrive on time with required identification (if applicable)
Failure to comply may result in cancellation or rescheduling at the company’s discretion.
- Right to Refuse Service
GeneScreen Mobile Diagnostics reserves the right to refuse or discontinue service if:
- Policies are not followed
- Required documentation is not provided
- Conditions compromise the integrity of the testing process
- Acknowledgment & Agreement
By booking an appointment, you:
- Confirm that you have read and understood all policies
- Agree to all fees, terms, and conditions outlined above
- Accept full responsibility for compliance with scheduling and attendance requirements
